My Sandisk Ultra 2GB card finally now works in my Sandisk 6-in-1 PC Card Adapter. 
I followed these steps:
Formatting the card using Windows XP
1. Insert the Sandisk 6-in-1 PC Card Adapter in the PC's PCMCIA slot.
2. Insert the card in the card adapter.
3. Double-click "My Computer".
4. Select the drive letter corresponding to the card.
5. Choose File > Properties.
6. Click the Tools tab.
7. Under Error checking, click the Check Now button.
8. Select both "Automatically fix file system errors" and "Scan for and attempt recovery of bad sectors".
9. Click Start.
Once I followed these steps, I formatted the card in the Treo, and I was able to copy files back and forth via card reader and Mac! I'm glad this finally worked since I prefer using the card reader via PCMCIA slot (which I don't use for anything else) instead of taking up a USB port.
Not to mention it's pretty fast -- very important if you're copying lots of big video files!
