    Have you actually opened the port on the router that the SlingBox needs to route the traffic to the Slingbox and back?

    Never had a problem via LAN.

    I guess I don't use my Slingbox enough to swap out my router. It works fairly well for everything else.

    I'm currently using a Netgear mr614 router, a cheapy.

    Find me a good deal on a new router, preferably Netgear (I like their web interface) and i'll upgrade.

    BTW, what is DMZ?

    Yes, port 5001 is being forwarded.

    I do occasionally have to re-add this port within port forwarding in the web-based router interface,

    DMZ allows you to forward all ports to a particular IP address within your network. Sometimes used for gaming or when you just need to put a computer/IP address outside of your router's protection.

    I'd say try updating the firmware on your router. You shouldn't be losing your port forwarding. It does sound like a router problem (unlike mine, which is a Slingbox problem, since even on my LAN side I have connectivity issues).
