What does a new 3G sim give you over an existing Cingular sim?
Is it merely a matter of data speed?

I am currently running my 750v (in UMTS mode) with my old Cingular sim without any issues. My last speed test to dslreports reported 308 kbps.

Is it worth replacing my sim with a new 3G sim?

There is no difference. The newer SIMs come with a fireball and says 3G on them. I called Cingular and asked. I was told its just cosmetics.

No, they only support rates up to EDGE.

Please excuse the ignorance, but would the UTMS capability permit use in the Japan 3g networks?

Yes, the 750v is one of only two devices I know that have tri-mode UMTS (850/1900/2100). In the U.S. Cingular uses 850/1900 frequencies (UMTS requires 2, upload and download). In Japan and Europe they use 1900/2100. So the 750v can work with UMTS world-wide.

Thanks. But the unlocked or Cing 750 also lists 850/1900/2100 utms so that will work in Japan also, right?

Well there is no unlocked or Cingular 750 yet, but I would expect the radios on them to be the same as the 750v. If so then yes they will work in Japan also.
